HIGH SCHOOL ENROLLMENT FORM

DANCE GUEST PERMISSION FORM
Homer-Center High School hosts some dances that permit our students to invite guests from other schools. In such cases Homer-Center students inviting non-HCHS students to dances at our school must submit a guest permission form within two days of the date of the sponsored dance. Guest permission forms can be downloaded from the attachment at the bottom of this page. Guest student school officials can fax these forms to the Homer-Center High School main office at 724-479-4236
 
EDUCATIONAL FAMILY TRIP REQUEST
Educational Trip Request form is for parents requesting their child be excused from school to attend a family trip educational in nature. Parents requesting an excused educational trip for their child  must submit the request at least two-weeks prior to the departure date of the trip. 
 
ENROLLMENT FORMS 
Students wishing to enroll at Homer-Center can download the enrollment forms at the bottom of this page and complete the packet to the best of their ability prior to presenting at the school to enroll.  New enrollments are processed by our guidance office throughout the summer during the school year as received. Questions regarding new enrollments should be directed to the high school guidance secretary, Erin Collins at [email protected] or at 724-479-8026 x 3126.
 
 
HOMEBOUND INSTRUCTION APPLICATION
Homebound Instruction Application is for students confined to home or hospital and unable to attend school due to a physical disability, illness, injury, urgent reasons, or for psychological or psychiatric reasons, as ordered by a physician or psychiatrist.
 
MEDICAL EVIDENCE STATUS-EXCUSE FORM
Medical Evidence Status-Excuse Forms are for students who have been placed on Medical Evidence for excessive absences. Students are placed on Medical Evidence Status after missing 10 or more days of school regardless if the absence has been deemed excused or unexcused.  Parents who believe their child has been placed on Medical Evidence in error should contact the school to discuss their concern.
