Health Services


  • Health plans need to be renewed each school year.  This includes Emergency Health Plans, Allergy Health plans, Asthma Health plans, Diabetic Health Plans, Seizure action plans, special procedure orders, Medical excuses(such as for physical education class), and Medication orders. Please return to the nurse as soon as possible. The forms are listed on this site. 
  • If your child has an order to self-carry and self-administer an inhaler, epi-pen or insulin, the above forms must be on file in the Nurse’s office. In addition, your child needs to demonstrate appropriate use to the nurse.  Please have your child stop to see the nurse.
  • A Medication administration consent form must be completed by a health care provider and signed by parent/guardian for any medication to be given at school.  This includes both over-the-counter and prescription medications.  All medication must be in an original bottle with appropriate labels. Medication received in baggies will not be administered. All medication must be kept in the nurse’s office with the exception of those with permission to carry inhaler, epi-pen or insulin.
  • Both families and school personnel want to keep children safe and healthy.  Unfortunately, children do get sick.  Please notify the school with any changes in phone numbers throughout the year in case your child becomes ill or an emergency occurs. 

Since ill and injured children feel most comfortable with family, we ask that you promptly pick up your child when we ask.  Another factor is that ill children may be exposing others at school to a contagious disease.

  • If your child has any acute or chronic health problems or surgery, please inform the school nurse.  Please update the changes throughout the school year including any medication changes your child is on at home or school.  

  • Please keep your child home from school if he/she has:
    • A temperature about 100.4F during the previous 24 hours (should be fever free without use of medication for 24 hours before returning to school.)
    • Vomiting or diarrhea during the prior 24 hours
    • Yellow or green nasal drainage associated with sore throat, nausea or fatigue
    • A persistent cough which has kept your child up at night
    • Signs or symptoms of Influenza, such as body aches, chills, headaches, fatigue
    • Please notify the school nurse if your child has an illness that is contagious, such as testing positive for Influenza.  This will help us detect illnesses in others sooner.
  • If your child is in Kindergarten, Grade 3, or Grade 7- please return the required Dental Form as soon as possible. 
  • If your child is in Kindergarten, Grade 6, or Grade 11- please return the required Physical Form as soon as possible.
